T2201 Disability Tax Credit Certificate
Individuals who have a severe and prolonged impairment in physical or mental functions, or their legal representative, use the T2201 form to apply for the
disability tax credit (DTC) by completing Part A of the form. Qualified practitioners use the form to certify the effects of the impairment by completing Part B of the form.
The disability amount is a non-refundable tax credit used to reduce income tax payable on your income tax and benefit return. This amount includes a supplement for persons under 18 years of age at the end of the year. All or part of this amount may be transferred to your spouse or common-law partner, or another supporting person.

Eligibility
You may be eligible if:
Your impairment in physical or mental functions lasted, or is it expected to last, for a continuous period of at least 12 months.
And one of the following applies:
-You are blind
-You receive life-sustaining therapy
-The effects of your impairment cause you to be markedly restricted at least 90% of the time in basic activities of daily living (such as speaking, hearing, walking, elimination, feeding, dressing, mental functions necessary for everyday life) even with the appropriate therapy, medication, and devices
-Or you are significantly restricted in two or more basic activities of daily living which cumulatively have the effect of being markedly restricted in one area.
