Child & Youth Mental Health Outreach Program
The Child and Youth Mental Health Outreach Program supports children/youth and their families to build skills, reduce and manage symptoms and increase opportunities for socialization. The goal of this program is to help individuals improve their personal, social, and vocational capabilities through community inclusion and access to resources. Strong, integrated case management with both the referring Mental Health Clinician and other significant members of the client’s personal and professional team is a key component. We work with the individual’s strengths and build service plans around what works best for them.
