Youth to Adult Transition Program
This youth service supports Youth 16-19 in the care of the Ministry of Children and Family Development to transition into independence. The program works with youth to develop a service plan to meet their goals towards independence and connecting to community resources. Program referrals are made by MCFD and VACFSS social workers through the Community Development & Rapid Response Coordinator with the MCFD Youth Connect Team.

Eligibility
Youth aged 16-19, who are in the care of MCFD or VACFSS. Program referrals are made by MCFD and VACFSS social workers directly to the Community Development & Rapid Response Coordinator with the MCFD Youth Connect Team
