Convalescent Care
The Convalescent Care Program is primarily for older adults with a chronic illness who require more time to recover before returning home following a period of hospitalization.

The publicly subsidized activation program is part of our short-term long term care services.

Following an acute illness, you may have lost much of your former mobility and functional independence. Our staff will assist you to regain your strength and mobility so you can safely carry out your daily living activities when you return home.

The average length of stay is seven weeks.

Eligibility
Clients receiving publicly subsidized convalescent care services must sign a consent for admission to the program and pay a daily or monthly charge. The current minimum client rate is $37.10 per day or $1,130.60 per month.

If you or your family believes you will experience serious financial hardship by paying this rate, you may apply for a temporary reduction.

Talk to your care team of your financial concerns; they will forward your request for consideration once you have been admitted to the program. If your application for hardship is approved, an adjusted daily rate will be charged.
